Choose your own support, keep more control, and lean on Silvergrace Care only where you need to.
Under Self-Managed Support at Home, you stay in the driver's seat. You choose who delivers your care, when they visit, and how your quarterly budget gets spent — Silvergrace Care stays involved only where the rules require it, and wherever you'd like a hand.
Most people choose this path for two reasons: more say over who's in their home, and better value from every dollar in their Support at Home budget.
You can mix registered support workers with independent workers you already know and trust — all within the same government-funded budget.
Bring in carers you already know, or ask us to help you find someone new.
Decide when services happen and how often — your budget, your timetable.
Track your quarterly funding yourself, with our help whenever you want a second set of eyes.
Worker screening, service agreements and payment processing stay in our hands, so nothing falls through the cracks.
Self-managing doesn't mean going it alone. Even though you're in charge day to day, our team is always a phone call away.
Explaining exactly what your funding can (and can't) cover
Helping you set up service agreements and find workers you can trust
Keeping an eye on your budget so you never overspend
Processing your invoices and payments on your behalf
Giving you clear advice whenever your needs or circumstances change
This path tends to suit people who:
One flat coordination fee, disclosed up front — never a mystery line on your statement.
As little or as much support from us as you want, whenever you want it.
Straightforward guidance and paperwork, without the jargon.
Change your arrangement whenever your circumstances change.
Talk to your care team directly — no call centres.
Silvergrace Care is a new provider, so we don't yet have a history of billed rates to publish the way an established provider might. Instead, here's exactly how your self-managed fees are calculated — no hidden math.
Example calculation, not an actual invoice: your support worker charges $50.00/hour → Silvergrace Care adds a $5.00 coordination fee (10%) → $55.00/hour is drawn from your Support at Home budget.
You can also draw on our own support workers, nurses and allied health professionals for any service — those are simply billed at our standard rates. View our full pricing →
The Australian Government allocates 10% of every participant's quarterly Support at Home budget to care management, whether your services are self-managed or fully managed by a provider. This isn't an extra charge from Silvergrace Care — it's built into how the program is funded, and it covers our compliance obligations: vetting the workers you choose, keeping your service agreement current, and processing your invoices accurately.
Prices and arrangements described here are indicative. Your final fees are confirmed in your individual Service Agreement and must align with your approved care plan and budget.
Cancellation terms are set directly between you and each support worker or provider you engage, and must not conflict with Support at Home rules.
When you engage your own worker or provider, Silvergrace Care applies a 10% administration fee per invoice to cover compliance, verification and processing.
Any rate increase from a worker or provider you've engaged is passed through at cost. You'll be notified before it takes effect, and it will appear on your monthly statement.
Support at Home funding can only be used for services on the official Support at Home Service List. It cannot cover rent, mortgage payments, general household bills, entertainment or alcohol, under any circumstances.
Any change to your services — type, frequency or duration — is discussed with you first and updated in your Service Agreement before it takes effect.
Every service, including any administration fee, is itemised on your monthly statement so you always know what you've been charged and why.
Silvergrace Care may decline or discontinue a service that's unsafe, outside the Support at Home Service List, or where compliance requirements can't be met.
Fees on this page are structured in line with the Aged Care Act 2024 and the Support at Home program rules. All prices are shown ex-GST where applicable, and every fee is itemised in your monthly statement.
